Advanced Practice Provider - NP or PA New

Bluestone Physician Services is a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive, and includes all members of the care team, patients and their families.  Join us in our team-based care as a Nurse Practitioner or Physician Assistant as we deliver on our promise to tailor primary care to patients that reside in group homes in the community of Maplewood and surrounding communities such as White Bear Lake and other east metro communities.

Position Description

• Provide on-site, team-oriented primary care to patients within the group home setting and assisted living communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 300+ patients (approximately 10-14 patients per/day)
• Ability to design and manage your schedule 
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers 
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as needed

 
Qualifications

• Graduate of NP program or PA program from an accredited school
• Must have and maintain a valid Minnesota License
• Professional clinical experience in primary care
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required
• Demonstrated compatibility with Bluestone’s mission and operating philosophies
• Demonstrated ability to read, write, speak, and understand the English language
• Travel throughout Twin Cities Metro required
• Covid-19 vaccines must be up to date by the date of hire. Accommodations will be considered.

• 2+ years clinical experience
